New York State Working Papers (Employment Certificates)
Employment Certificates (Working Papers) for teens can be obtained in the main office at Fishers Island School by appointment only. Please call the school at 631-788-7444 / 631-357-4692 (Christian cell) or email c.arsenault@fischool.com if you have questions or to make an appointment. All minors (14-17 years old) who work are required to have an employment certificate.

Physical exam completed and signed by a physician within the last 12 months
